We have moved to a new online system.  Once you’ve set up your initial appointment with Kara, please be on the lookout for an email from “wellness”.  This will get you setup in our online system.  Everything you  and Kara need will be right there.  You have 24 hours from the time your email is sent to complete your paperwork.  If you need more time, please complete just the “Cancellation Policy” and let Kara know you need some extra time.
